Insurance Participation

At Menla Healthcare LLC we accept assignment from many managed care and traditional insurance plans. This means that we will bill these entities for any fees that we would charge you for services we provide and have agreed to accept their allowed amount. Most health plans also require a patient deductible and co-payment, which we will collect from you at the time of service. We accept cash, checks, and most major credit cards at our office.

Please bring your insurance card with you for each visit and notify our front desk anytime your insurance coverage changes. We participate with the following plans:

If you have a health plan that is not on this list, we will charge our regular fee, and as a courtesy to you, bill your health plan directly for reimbursement. You will be responsible for any fees remaining after your insurance carrier has paid their share. If you don't have health insurance, we will be glad to see you. If needed, we can arrange a payment plan for you.
